Report To Contact the lead preceptor or contact person one month prior to your preceptorship starting date. The first day of your rotation (Monday) is for traveling to your preceptorship site to get settled with housing, taking part in any site orientation and at 6 p.m. taking part in a phone orientation in which you will need computer access. You will be informed of the remaining two phone orientations from Dr. Paul Hunter, Course Director. In most cases you will begin clinical work on the second day (Tuesday) of the rotation, however, please check with your site as some students may be on call the first night of your preceptorship. Your background check information and immunization records can be found in OASIS under Academic History/My Med School. It is your responsibility to copy that information and send it to your site coordinator or preceptor via email before your preceptorship starts.
Housing A guest cabin at Dr. Vitale's home is available, which is a bedroom, 3/4 bath and living room. The rooms overlook Big Wood Lake (520 acres). Many outdoor activities are available to do. Outdoor grill and recreational equipment (e.g. kayak, pontoon, and hammock) available for your use . Wireless InterNet access (DSL) is and Sattelite TV provided. The guest area has its own microwave and refrigerator. Laundry machines are available in Dr. Vitale's home.
FOOD: All meals are FREE in the hospital cafeteria.
Parking TRANSPORTATION NEEDS: Car is necessary (provided by student). Free parking at hospital and clinics.

Upon completion of the 4th Year Preceptorship, students will:
1. Understand the complexities of a multidisciplinary health care system
2. Develop proficiency in providing patient care in a community setting
3. Apply principles of preventive medicine to clinical care
4. Recognize the impact of health care and reimbursement systems on the delivery of health care
5. Expand upon their personal and professional growth and development
6. Recognize the effect of quality measurement and improvement on clinical decision-making, cost of care, and patient satisfaction

Patient Care (Problem solving and Clinical Skills)
Students are expected to provide patient care that is compassionate, appropriate, and effective for the treatment of health problems and the promotion of health.

Students are expected to investigate and evaluate their patient care practices by appraisal and assimilation of scientific evidence.

Systems Based Practice -
Students are expected to demonstrate an awareness of the larger context and system of health care and effectively call on system resources to provide optimal care.
1. Use and refer patients to appropriate community-based healthcare resources

3. Describe your preceptor’s relationship to the healthcare system in which (s)he practices
4. Describe the roles of federal, state and private programs that pay for health care within the US healthcare system

Content The student will generally work with a single provider (primary care or specialist) most days with the majority of the rotation devoted to general family practice. The rotation intends to expose the student not only to the usual range of family medicine health issues, but also to the concepts and delivery of community oriented primary care. It is expected that the student will understand the community's broader health needs and may include time with other allied health organizations such as public health. The family medicine experience will introduce the student to a variety of clinical problems with the most intensive patients being stabilized and transferred. Opportunities to work in the ER, nursing home, Public Health Department and to assist with surgeries are available. TRIBAL Clinic visit is also possible.
Prerequisites
Methods Burnett Medical Center is a non-profit, community run health center dedicated to providing a broad spectrum of primary health care to an underserved population. The hospital is a 17 bed hospital with an average daily census of 5 - 7 patients. A new clinic and emergency room are under construction (to be finished by the end of 2008). The clinic and hospital is staffed with 4 family physicians, Blaise Vitale, MD, Tim Novick, MD, Adeola Jolayemi, MD and Vesna Radivojevic, MD 2 physician's assistants, Kathy Nathan and Julie Anderson, 2 nurse practitioners, Melinda Deye and Adrianna Addison and a nurse midwife, Deb Hammer. The hospital also has many visiting specialists including general surgery, ENT, OB/GYN, ophthalmology, neurology, cardiology, orthopedics, podiatry, pulmonolgy, urology and radiology. 2 additional physicians also staff the emergency room on weekends. There is also a 53 bed nursing home attached to the facility.
Grantsburg is a community of ~1300 people near the northwest border of Wisconsin. The community is located ~85 miles northeast of Minneapolis/St. Paul and ~100 miles south of Duluth. It is the largest town in Burnett County, which is the primary living area of our patients. There are large tracts of public land available for recreation including Crex Meadows (the largest wildlife area in the state) and the St. Croix Scenic Riverway. The community is poor but is growing. Dr. Vitale came here through the national Health Service Corps.
Materials Provided
Expectations The student is expected to take call at the hospital when Dr. Vitale is on call (usually 1 - 2 times weekly).
Evaluating A mid-rotation and cumulative evaluation will be completed by the preceptor based on all attendings input involved in the clinical and academic experiences.
