Objectives
|
1) Medical Knowledge:
Understand the evidence-based medicine behind the care management initiative protocols.
2) Patient Care:
Appropriately determine aspects of patient-centered medical home that are impacted most by quality care initiatives.
Determine which factors impact safe medication use and compliance in the outpatient setting.
Understand the importance of medication reconciliation at each visit as it relates to patient safety and adherence.
3) Interpersonal and Communication Skills:
Communicate effectively and work collaboratively with team members who are part of clinic and system quality measures in care management.
Partner effectively and appropriately with the patient and her family/support, especially those with low health literacy.
4) Professionalism
Develop ability to deal professionally with patients, their families, facility staff and attending physicians.
Understand the need to be attention to administrative aspects of their duties.
Be committed to personal integrity and honesty.
Demonstration of:
Genuine interest in helping patients with health needs regardless of their cultural or economic background
Sensitivity and responsiveness to a diverse patient population, including but not limited to diversity in gender, age, culture, race, religion, disabilities, and sexual orientation.
Prompt attendance at rotation with appropriate appearance and behavior.
Appropriate completion of assigned clinical tasks and records.
Understanding and compliance of by laws and guidelines governing medical care such as HIPPA.
5) Practice-based Learning and Improvement:
Assess principles of continuous quality improvement to the care of patients related to a chosen care management initiative in a particular setting of choice.

Content
|
Students will work with clinical faculty and care management staff to explore and assess the health of the metro Milwaukee patient population as it relates to chronic disease and prevention through care management and health care coordination. This two week course will engage the student in the many aspects of quality and health care management through meetings with leaders in Aurora Health Care quality programs, site visits to a spectrum of clinical practice models participating in care management, and use of on-line programs, independent reading and reflection.
