This experience was developed in collaboration with the UW Geriatrics Fellowship and Education Committee, the United Way of Dane County, VA Transitional Care program personnel, and Pharmacists and Social Workers at the Oakwood Retirement Community.

Objectives
|
1. Work effectively in an interdisciplinary team (Teamwork)

Content
|
This experience will emphasize a systems-based practice (SBP) perspective. As such students will gain an awareness of and responsiveness to the larger context and system of health care and the ability to effectively call on system resources to provide optimal health care. This elective will highlight many key areas of geriatrics practice and SBP organized around the ABIM mnemonic: TACTICS (Teamwork, Advocacy, Coordination, Technology, Improvement tools/ skills, Cost and Safety) including teamwork/collaboration, the care continuum, the social context of care and risk avoidance of iatrogenic events. It will also instruct students about how Microsystems, Mesosystems and Macrosystems interrelate and influence patient care and outcomes.

Methods
|
Time will be divided between the Acute Care for Elders inpatient team at the UWHC, the VA and UWHC Transitional Care programs, and several community-based settings that emphasize safe aging and injury prevention. The community experiences include post-discharge home visits, observation of health risk appraisals conducted at Oakwood Village, an area retirement community, and involvement in the United Way of Dane Countys Safe and Healthy Aging Initiative.

Evaluating
|
The evaluation methods will include multisource feedback forms, direct observation tools that incorporate behavior checklists, a reflective essay and several web-based pre/post test modules. Methods will include use of a series of validated Objective Structured Video Exams (OSVEs) developed by colleagues at the Medical College of Wisconsin that use video captions to assess students skills and knowledge in core concepts of geriatrics and systems issues.
