The rotation occurs in the Madison area and occasionally in the surrounding community. Most training is done in VA and UW clinics. Students may also participate in nursing home rounds and make home visits with providers. A car is helpful for transportation, but not required.

Objectives
|
1) Enhance ambulatory care skills for taking care of older adults.
2) Participate on an in-patient consult team focusing on care of hospitalized elders.
3) Develop skills to care for older adults in long term care settings, such as nursing homes.

Content
|
1) Demonstrate knowledge of basic approaches in recognizing and managing common geriatric syndromes such as cognitive impairment, depression and falls.
2) Receive instruction and demonstrate proficiency in administering clinic based assessment tools to identify cognitive impairment, gait disturbances, functional status and depression.
3) Acquire strategies for appropriate prescribing and medication adjustments in order to reduce medication related problems and adverse effects.
4) Recommend age-appropriate preventive care for older adults including identifying health behaviors, life style changes and patient safety strategies that promote successful aging.

Prerequisites
|
4th year medical student standing or completion of 632-812 (3rd year medicine clerkship).

Methods
|
Instruction is based on team based discussions about patients’ health care treatment and management. Clinic visits will vary. Weekly teaching conferences are held on Tuesdays including a lecture 7:30-8:30a and journal club 12:00-1:00p in the GRECC (Geriatric Research Education and Clinical Center), 4th fl VAH.

Expectations
|
Students will have a variety of patient encounters, experience multiple training sites and interact with a number of different faculty members in addition to members of the inter-disciplinary team.
|
Evaluating
|
Informal feedback will be offered throughout the rotation and a more formal OASIS form evaluation with a supervising faculty member will be held at the conclusion of the rotation. A self evaluation will be also be part of the experience.
